What is the Two Rivers project?
The Two Rivers project is a two year project, in which our Active Wellbeing service will be working closely with the Most Active County (MAC) Partnership and the Two Rivers Medical Centre in Ipswich, to support older adults who are physically inactive to become more physically active.
If you are a patient registered at Two Rivers Medical Centre and are looking for help to get more physically active please complete the sign up form below.
Please note, as a result of the Covid-19 outbreak, our Health and Wellbeing team have adapted their usual service. Referrals into the Active Wellbeing service can still be made, however all existing and new patients will now be offered telephone support, instead of the usual face-to-face consultations. This is temporary and in the meantime, the team are preparing for the launch of new projects and consultations which can go ahead once the current social distancing rules are lifted by the government.
