Help us to shape our new 5 year plan for Children & Young People

At Active Suffolk we are currently working to revise our offer to Children and Young People across the county by developing a new five year plan. As part of this work, we have teamed up with Healthwatch Suffolk to carry out some research and form a co-production so that local people can have their say!
Rather than making the decisions ourselves, we are working with Healthwatch Suffolk to gather insight from children and young people from school years, year 1 – 11 and we need your help.
The survey which has been created as part of this project is to be completed by parents and/or carers of children from year 1 – 3 and children and young people from years 3 - 11 in Suffolk. The survey should take no more than 5 minutes to complete and all responses are anonymous and not linked to an individual's contact details or name in any way.
As part of this piece of work, two schools in Suffolk will be issued £300 of vouchers which can be used on PE and Sports equipment if they have 20 or more surveys completed and submitted from parents or pupils from the school.

In addition to the survey, we are also inviting people aged 14+ across Suffolk to work on a co-production to help shape the five year plans.
